CMS-1500 Medical Narrative Attachment Update

Health care providers are strongly encouraged to include causal relationship, work status and temporary impairment on most medical narrative attachments to the CMS-1500.

There are several instances, however, in which this information is not as critical to the payer, or the Board, and it is not needed to properly adjudicate the medical bill. Certain provider types (acupuncturists, physical therapists and occupational therapists) are not permitted to give opinions on impairment, and physician assistants must have the direct supervision of a physician when opining on initial impairment.

The Board notified payers on November 18, 2022, that they should not routinely file a Notice of Objection to a Payment of a Bill for Treatment Provided (Form C-8.1B) to deny payment to health care providers solely based on the absence of one or more of these three elements.
