ROMILLA ANWAR, M.D. (Authorization No. 195639-0B)

Please be advised that, effective April 12, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Surrender Order (BPMC No. 10-55), ordered the surrender of the medical license of Romilla Anwar, M.D. No current business address is available for Dr. Anwar.

Dr. Anwar is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Anwar for treatment rendered or examinations conducted prior to April 12, 2010, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date. Requests to cross-examine Dr. Anwar regarding treatment she rendered or examinations she conducted prior to April 12, 2010, should not be denied due to the surrender of her license to practice medicine.

SERGIO A. CASTILLO, M.D. (Authorization No. 176200-4W)

Please be advised that, effective April 13, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Consent Order (BPMC No. 10-57), has suspended the medical license of Sergio A. Castillo, M.D., for nine (9) months. No current business address is available for Dr. Castillo.

Dr. Castillo is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Castillo for treatment rendered prior to April 13, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Castillo regarding treatment he rendered prior to April 13, 2010, should not be denied due to the suspension of his license to practice medicine.

RICHARD P. KULAK, D.O. (Authorization No. 210974-2B)

Please be advised that, effective April 12, 2010, the New York State Department of Health, Board for Professional Medical Conduct, issued an Order for Non-Practice of Medicine, by which Richard P. Kulak, D.O., must cease all practice of medicine in New York State pending the final disposition of an investigation currently being conducted by the Office of Professional Medical Conduct. No current business address is available for Dr. Kulak.

Dr. Kulak is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kulak for treatment rendered or examinations conducted prior to April 12, 2010, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date until further notice. Requests to cross-examine Dr. Kulak regarding treatment he rendered or examinations he conducted prior to April 12, 2010, should not be denied due to his indefinite cessation from the practice of medicine.

MICHAEL J. HOROWITZ, D.C. (Authorization No. C07885-9W)

Please be advised that, effective November 30, 2009, the registration of Michael Horowitz, D.C.'s license to practice chiropractic expired. Previously, effective January 18, 2008, Dr. Horowitz' authorization to treat injured workers in the New York State workers' compensation system had been suspended through January 17, 2009 (see Subject Number 046-219). No current business address is available for Dr. Horowitz.

Dr. Horowitz is prohibited from rendering chiropractic treatment and care to individuals who have suffered work-related injuries or illnesses. Chiropractic reports submitted by Dr. Horowitz for treatment rendered prior to November 30, 2009, are valid except during the suspension period of January 18, 2008 through January 17, 2009. Reports submitted by Dr. Horowitz for treatment rendered on or after November 30, 2009 are invalid. Requests to cross-examine Dr. Horowitz regarding treatment he rendered prior to January 18, 2008, or between January 18, 2009, and November 29, 2009, should not be denied due to the inactive status of his license to practice chiropractic or his suspension from the New York State workers' compensation system.
