Please be advised that, effective January 18, 2008, Michael Horowitz, D.C.'s authorization (Number C07885-9W) to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13-l(12). Dr. Horowitz's address is 80 Wall Street, Suite 200, New York, New York 10005-3601. The suspension is effective until January 17, 2009.

While the suspension is in effect, Dr. Horowitz is prohibited from rendering chiropractic treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to January 18, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Horowitz regarding treatment he rendered prior to January 18, 2008, should not be denied due to this suspension.
