Please be advised that Dr. Michaelene Callahan has resigned her authorization to perform independent medical examinations in the New York State workers’ compensation system effective October 9, 2015. Dr. Callahan’s last known practice address is 127 Livingston Avenue, New Brunswick, New Jersey 08901.

Dr. Callahan is prohibited from performing independent medical examinations of injured workers. Reports submitted by Dr. Callahan for independent medical examinations performed prior to October 9, 2015, are valid, but are invalid for independent medical examinations performed on or after that date. Requests to cross-examine Dr. Callahan for independent medical examinations performed prior to October 9, 2015, should not be denied due to her resignation.
