Please be advised that Dr. Pedro Luis Gonzalez-Torres has resigned his authorization to treat injured workers in the New York State workers’ compensation system effective September 19, 2015. Dr. Gonzalez-Torres’ last known practice addresses are: 521 Route 111, Suite 204, Hauppauge, New York 11788; 763 Larkfield Road, Commack, New York 11725; and 150 E. Sunrise Highway, Lindenhurst, New York 11757.

Dr. Gonzalez-Torres is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Gonzalez-Torres for services rendered prior to September 19, 2015, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Gonzalez-Torres for services rendered prior to September 19, 2015, should not be denied due to his resignation.
