Please be advised that Dr. Arthur Menken's authorization to treat injured workers in the New York State workers' compensation system has been suspended indefinitely effective February 4, 2013. Furthermore, Dr. Menken's authorization to perform independent medical examinations has been removed effective January 25, 2013. Dr. Menken's last known business address is 21 Reade Place, Poughkeepsie, NY, 12601.

Dr. Menken is prohibited from rendering treatment and care to injured workers while his authorization is suspended. Reports submitted by Dr. Menken for treatment rendered prior to February 4, 2013, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Menken for treatment rendered prior to February 4, 2013, should not be denied due to the suspension of his authorization.

Dr. Menken is prohibited from performing independent medical examinations. Reports submitted by Dr. Menken for independent medical examinations performed prior to January 25, 2013, are valid, but are invalid for any independent medical examinations performed on or after that date. Requests to cross-examine Dr. Menken for independent medical examinations performed prior to January 25, 2013, should not be denied due to this removal.
