Please be advised that, effective June 22, 2010, David E. Rosenberg, M.D., voluntarily resigned his authorization to conduct independent medical examinations of individuals who have suffered work-related injuries and illnesses. Dr. Rosenberg's business address is 63 New Main Street, Haverstraw, New York 10927.

Dr. Rosenberg is prohibited from conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Reports submitted by Dr. Rosenberg for examinations conducted prior to June 22, 2010, are valid, but are invalid for any examinations conducted on or after that date. Requests to cross-examine Dr. Rosenberg regarding examinations he conducted prior to June 22, 2010, should not be denied due to the voluntary resignation of his authorization.
