June 3, 2010
Authorization No. P04818-2B
Please be advised that, effective June 1, 2010, the Chair of the New York State Workers' Compensation Board determined, pursuant to Workers' Compensation Law Sections 13-d(2) and 13-k(10), to permanently revoke the authorization of Alan J. Cantor, D.P.M. Previously, the Office of the Professions listed Dr. Cantor's license to practice podiatry as suspended as of February 16, 2010, and his license remains suspended as of June 1, 2010. No business address is available for Dr. Cantor.
Dr. Cantor is prohibited from rendering podiatric treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Cantor for treatment rendered or examinations conducted prior to February 16, 2010, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date. Requests to cross-examine Dr. Cantor regarding treatment he rendered or examinations he conducted prior to February 16, 2010, should not be denied due to the suspension of his license to practice podiatry or the permanent revocation of his authorization.
Any questions regarding this matter should be referred to the Office of General Counsel at (518) 486-9564.
