Jeffrey A. Gelber, M.D. (Authorization No. 129283-8W)

Please be advised that, by Consent Order (BPMC No. 10-62), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective April 26, 2010, Jeffrey A. Gelber, M.D., admitted guilt to engaging in professional misconduct.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Gelber from the list of authorized physicians by revoking his authorization, effective May 14, 2010. Dr. Gelber's reported address on the BPMC Consent Order is 3595 Eggert Road, Orchard Park, New York 14127.

Dr. Gelber is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gelber for treatment rendered prior to May 14, 2010, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Gelber regarding treatment he rendered prior to May 14, 2010, should not be denied due to the removal of his name from the list of authorized physicians.

Muhammad Hena, M.D. (Authorization No. 113461-8W)

Please be advised that, by Determination and Order (BPMC No. 09-205), as modified on appeal to the Administrative Review Board for Professional Medical Conduct of the New York State Department of Health, effective May 3, 2010, Muhammad Hena, M.D., was found guilty of engaging in professional misconduct.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Hena from the list of authorized physicians by revoking his authorization, effective May 14, 2010. Dr. Hena's reported address on the BPMC Determination and Order is 4 Atrium Drive, Suite 200, Albany, New York 12205.

Dr. Hena is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Hena for treatment rendered prior to May 14, 2010, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Hena regarding treatment he rendered prior to May 14, 2010, should not be denied due to the removal of his name from the list of authorized physicians.

Jae Ook Park, M.D. (Authorization No. 130080-5B)

Please be advised that, by Determination and Order (BPMC No. 10-63), issued by a hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective April 27, 2010, Jae Ook Park, M.D., was found guilty of engaging in professional misconduct.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Park from the list of authorized physicians by revoking his authorization, effective May 23, 2010. No current address information is available for Dr. Park.

As announced in Subject No. 046-425, Dr. Park was indefinitely prohibited as of July 31, 2009, from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Park for treatment rendered or examinations performed prior to July 31, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Park regarding treatment he rendered or examinations he performed prior to July 31, 2009, should not be denied due to the removal of his name from the list of authorized physicians.
