BENJAMIN EUN LEE, M.D. (Authorization No. 130038-3W)

Please be advised that, effective February 17, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Consent Order (BPMC No. 10-22), has precluded, pursuant to N.Y. Public Health Law § 230-a, Benjamin Eun Lee, M.D., from all patient contact and any practice of medicine, clinical or otherwise, in New York State. Further, the status of Dr. Lee's medical license was reported by the State Education Department as inactive, effective February 1, 2010. No current business address is available for Dr. Lee.

Dr. Lee is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lee for treatment rendered prior to February 1, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Lee regarding treatment he rendered prior to February 1, 2010, should not be denied due to the limitation on his practice of medicine or the inactive status of his license to practice medicine.

DUK HEE LEE, M.D. (Authorization No. 147598-7W)

Please be advised that, effective March 23, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Consent Order (BPMC No. 10-39), has precluded, pursuant to N.Y. Public Health Law § 230-a, Duk Hee Lee, M.D., from all patient contact and any practice of medicine, clinical or otherwise, in New York State, and his license to practice medicine became inactive. No current business address is available for Dr. Lee.

Dr. Lee is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lee for treatment rendered prior to March 23, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Lee regarding treatment he rendered prior to March 23, 2010, should not be denied due to the limitation on his practice of medicine or the inactive status of his license to practice medicine.

OVID O. NEULANDER, M.D. (Authorization No. 144705-1B)

The information regarding Dr. Ovid O. Neulander has been revised from the original release on April 2, 2010.

Please be advised that the Order of Limitation of Medical Practice issued against Ovid O. Neulander, M.D., as noted in the original version of this Subject Number issued on April 2, 2010, has been withdrawn by the New York State Department of Health, Board for Professional Medical Conduct, effective January 11, 2011. Dr. Neulander remains fully authorized to treat injured workers and to perform independent medical examinations in the New York State workers' compensation system. Therefore, Medical reports for treatment rendered or examinations conducted by Dr. Neulander on and after March 19, 2010, are valid.

DONALD OROFINO, M.D. (Authorization No. 099254-5W)

Please be advised that, effective March 12, 2010, the New York State Department of Health, Board for Professional Medical Conduct, issued an Interim Order of Conditions pursuant to N.Y. Public Health Law § 230, by which Donald Orofino, M.D., is precluded from all practice of medicine in New York State, which will take effect on April 26, 2010. Dr. Orofino's business address is 100 Manetto Hill Road, Suite 309, Plainview, New York 11803.

Dr. Orofino is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Orofino for treatment rendered prior to April 26, 2010, are valid, but are invalid for any treatment rendered on or after that date until further notice. Requests to cross-examine Dr. Orofino regarding treatment he rendered prior to April 26, 2010, should not be denied due to his indefinite preclusion from the practice of medicine.
