Please be advised that, effective February 1, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Modification Order (BPMC No. 99-182), adopted the agreement of Joseph D. Beasley, M.D., to never reactivate his registration or reapply for a license to practice medicine in the State of New York. The modification of Consent Order BPMC No. 99-182 does not constitute a new disciplinary action. No current business address is available for Dr. Beasley.
Dr. Beasley is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Previously, by Non-practice Order, Dr. Gregory was precluded from the practice of medicine, effective September 22, 2006, until final disposition of the then investigation. Dr. Gregory is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gregory for treatment rendered prior to September 22, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. O'Connell regarding treatment he rendered prior to February 23, 2010, should not be denied due to the permanent inactive status of his license to practice medicine.
ALEXANDER ROZENBERG, M.D. (Authorization No. 207165-2W)
Please be advised that, effective March 8, 2010, the Commissioner of the New York State Department of Health, through the State Board for Professional Medical Conduct, issued a Commissioner's Order and Notice of Referral Proceeding whereby Alexander Rozenberg, M.D., may not practice medicine in New York State or in any jurisdiction where the practice of medicine is based solely upon a New York license. This Order remains in effect unless modified by the New York State Commissioner of Health. Previously, on March 14, 2007, Dr. Rozenberg agreed to refrain from treating individuals who have suffered work-related injuries and illnesses. No current business address is available for Dr. Rozenberg.
Dr. Rozenberg is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rozenberg for treatment rendered prior to March 14, 2007, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Rozenberg regarding treatment he rendered prior to March 14, 2007, should not be denied due to his agreement with the Workers' Compensation Board or his indefinite preclusion from the practice of medicine.
