Please be advised that by Surrender Order (BPMC No. 10-23), effective February 18, 2010, the Department of Health's Board for Professional Medical Conduct accepted and adopted the surrender of the license to practice medicine of Ashraf Azeb, M.D. Dr. Azeb's last known business address is 563 Hambley Boulevard, Pikeville, Kentucky, 41501.

Effective February 18, 2010, Dr. Azeb is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Azeb for medical treatment or examinations rendered prior to February 18, 2010, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Azeb regarding medical treatment he rendered or examinations he conducted prior to February 18, 2010, should not be denied due to the surrender of his license to practice medicine.
