Effective December 21, 2009, JMC Associates, Inc., voluntarily withdrew its designation as an entity registered to derive income from independent medical examinations (IME entity) in workers' compensation cases. This withdrawal is made pursuant to Section 13-n of the Workers' Compensation Law and 12 NYCRR 300.2(e) and is permanent. The entity's last known address is 2034 East 74th Street, Brooklyn, New York 11234.

As of December 21, 2009, JMC Associates, Inc., is permanently prohibited from deriving income from independent medical examinations in workers' compensation cases, and from participating as an IME entity in the New York State workers' compensation system. Independent medical examinations conducted prior to December 21, 2009, are valid. Income derived by JMC Associates, Inc., before the effective resignation date of December 21, 2009, is permitted.
