Subject Numbers Regarding Health Provider Authorizations

Please be advised that, effective December 10, 2009, Anil K. Tripathy, M.D., voluntarily resigned his authorization to treat individuals who have suffered work-related injuries or illnesses. Previously, Dr. Tripathy's license to practice medicine was suspended, effective November 13, 2009, for six months, which was published in Subject Number 046-383. No current business address is available for Dr. Tripathy.

Dr. Tripathy is prohibited from rendering medical treatment and other care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Tripathy for treatment rendered prior to November 13, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Tripathy regarding treatment he rendered prior to November 13, 2009, should not be denied due to the suspension of his license to practice medicine or the resignation of his authorization.
