Please be advised that, effective November 17, 2009, Jay Lippman, M.D., voluntarily resigned his authorization to treat individuals who have suffered work-related injuries or illnesses. Dr. Lippman's business address is 828 Pelhamdale Avenue, New Rochelle, NY, 10801.

Dr. Lippman is prohibited from rendering medical treatment and other care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lippman for treatment rendered prior to November 17, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Lippman regarding treatment he rendered prior to November 17, 2009, should not be denied due to the resignation of his authorization.
