Please be advised that, by Modification Order (BPMC No. 08-132), effective October 20, 2009, the New York State Department of Health, State Board for Professional Medical Conduct, accepted the agreement of Allan Roger Serviss, M.D., that he will never activate his registration or reapply for a license to practice medicine in New York State. No current business address is available for Dr. Serviss.

Effective October 20, 2009, Dr. Serviss is precluded from rendering medical treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Serviss for treatment or examinations rendered prior to October 20, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Serviss regarding treatment he rendered or examinations he conducted prior to October 20, 2009, should not be denied due to the inactivation of his medical license.
