Please be advised that, by Consent Order (BPMC 09-140), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective July 31, 2009, Bruce P. Klein, M.D., admitted guilt to the charge of negligence on more than one occasion. By Subject Number 046-362, published September 28, 2009, Dr. Klein's name was removed from the list of authorized physicians, effective September 22, 2009. By decision of the Chair, Dr. Klein's removal is withdrawn, effective October 1, 2009, and a temporary suspension for the period of September 22, 2009, to October 1, 2009, has been instituted. Dr. Klein's business address is 1160 Corporate Drive, Suite 400, Farmington, New York 14425.

Dr. Klein was prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses during the period of September 22, 2009, to October 1, 2009. Medical reports submitted by Dr. Klein for treatment rendered prior to September 22, 2009, and after October 1, 2009, are valid, but are invalid for any treatment from September 22, 2009, to October 1, 2009. Requests to cross-examine Dr. Klein regarding treatment he rendered prior to September 22, 2009, or after October 1, 2009, should not be denied due to the temporary suspension of his authorization.
