Please be advised that, effective October 1, 2009, Edward P. Foley, D.O., voluntarily resigned his authorization to treat individuals who have suffered work-related injuries or illnesses. Dr. Foley's business address is Universal Healthcare, 1995 Ferndale Road, Castleton, New York 12033.

Dr. Foley is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Foley for treatment rendered prior to October 1, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Foley regarding treatment he rendered prior to October 1, 2009, should not be denied due to the resignation of his authorization.
