Please be advised that, effective July 29, 2009, the New York State Department of Health, State Board for Professional Medical Conduct, issued a Nondisciplinary Order of Conditions pursuant to New York State Public Health Law Section 230 for the medical license of Joel D. Baskoff, M.D. Previously, Dr. Baskoff's license registration became inactive on January 1, 2009. No current business address is available for Dr. Baskoff.

Effective January 1, 2009, Dr. Baskoff is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Baskoff for treatment or examinations rendered prior to January 1, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Baskoff regarding treatment or examinations he rendered prior to January 1, 2009, should not be denied due to the inactive status of his license.
