Please be advised that, effective July 14, 2009, Joyce A. Klindt, M.D., permanently surrendered her license to practice medicine in the State of New York pursuant to New York State Public Health Law Section 230.13. Dr. Klindt voluntarily resigned her authorization to treat individuals who have suffered from work-related injuries or illnesses, effective August 25, 2009. No current business address is available for Dr. Klindt.

Dr. Klindt is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Klindt for treatment rendered prior to July 14, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Klindt regarding treatment she rendered prior to July 14, 2009, should not be denied due to the resignation of her authorization or surrender of her license to practice medicine.
