Please be advised that the authorization of Stewart Schwartz, D.O., to treat workers' compensation claimants has been temporarily suspended for one year, effective August 25, 2009, to August 24, 2010. His last known business address is 260 Garth Street, Suite 2H5, Scarsdale, New York 10583.

Dr. Schwartz is prohibited from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Schwartz for treatment rendered prior to August 25, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Schwartz regarding treatment he rendered prior to August 25, 2009, should not be denied due to the temporary suspension of his authorization.
