Please be advised that, by Determination and Order (BPMC No. 09-115), effective June 24, 2009, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), found Khaled Omar Elbanna, M.D., guilty of obtaining a license fraudulently; practicing the profession fraudulently; having been disciplined by the Florida State Board of Medicine for failing to appear before the Board for application issues; having been disciplined by the Illinois State Division of Professional Regulation for practicing medicine without a license and making or filing a false report. Effective June 24, 2009, Dr. Elbanna's license to practice medicine in the State of New York was suspended for one year. This action has been appealed to the Administrative Review Board; however, the period of suspension remains in effect until the Review Board issues an order.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Elbanna from the list of authorized physicians by revoking his authorization, effective August 7, 2009. Dr. Elbanna's last known business address is 25-43 Steinway Street, Astoria, New York 11103.
Dr. Elbanna is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Elbanna for treatment rendered prior to June 24, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Elbanna regarding treatment he rendered prior to June 24, 2009, should not be denied due to the removal of his name from the list of authorized physicians.
