Please be advised that, by Decision and Judgment of Supreme Court Justice Eugene P. Devine, dated May 21, 2009, and entered on June 11, 2009, the September 16, 2008, removal of Dr. James M. Liguori's name from the list of authorized physicians (see Subject No. 046-258) was annulled, and the matter was remitted to the New York State Workers' Compensation Board for further proceedings not inconsistent with said Decision and Judgment. Accordingly and pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to once again remove Dr. Liguori's name from the list of authorized physicians, effective June 29, 2009. Dr. Liguori's business address is 1999 Marcus Avenue, Suite M11, New Hyde Park, New York 11042.

Dr. Liguori is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Liguori for treatment rendered prior to June 29, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after June 29, 2009. Requests to cross-examine Dr. Liguori regarding treatment he rendered prior to June 29, 2009, should not be denied due to the removal of his name from the list of authorized physicians.
