Authorization No. 173856-6W
Please be advised that, effective April 1, 2009, the New York State Department of Health's State Board adopted Consent Order (BPMC No. 09-57), and suspended Nisar Ahmed Piracha, M.D.'s license to practice medicine. Effective May 27, 2009, Dr. Piracha voluntarily resigned his authorization to treat workers' compensation claimants. No current business address is available for Dr. Piracha.
Dr. Piracha is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Piracha for treatment rendered prior to April 1, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Piracha regarding treatment he rendered prior to April 1, 2009, should not be denied due to his license suspension or the resignation of his authorization.
Any questions regarding this matter should be referred to the Office of General Counsel at (518) 486-9564.
