Please be advised that, effective February 17, 2009, a Hearing Committee of the New York State Department of Health's State Board for Professional Medical Conduct revoked the medical license of Constancio P. Tiglao, M.D. Effective April 16, 2009, Dr. Tiglao voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 11 Ralph Place, Suite 214, Staten Island, New York 10304.

Dr. Tiglao is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Tiglao for treatment rendered prior to February 17, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Tiglao regarding treatment he rendered prior to February 17, 2009, should not be denied due to the revocation of his license to practice medicine or this resignation.
