Subject Numbers Regarding Health Provider Authorizations

John Rague Mangiardi, M.D. (Authorization No. 136921-4W)

Please be advised, effective December 30, 2008, Dr. Mangiardi surrendered his license to practice medicine in the State of New York. Effective January 16, 2009, Dr. Mangiardi voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 54 Doubling Road, Greenwich, Connecticut 06830.

Dr. Mangiardi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Mangiardi for treatment rendered prior to December 30, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Mangiardi regarding treatment he rendered prior to December 30, 2008, should not be denied due to the surrender of his medical license or his resignation.

Stuart L. Resch, M.D. (Authorization No. 118987-7W)

Effective January 27, 2009, Dr. Resch voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 3530 Henry Hudson Parkway, Apt. 4K, Bronx, New York 10463.

Dr. Resch is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Resch for treatment rendered prior to January 27, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Resch regarding treatment he rendered prior to January 27, 2009, should not be denied due to his resignation.
