Please be advised that Angelo C. Rizzo, D.O.'s license to practice medicine in the State of New York was restricted to prohibit the clinical practice of medicine, effective October 22, 2008. His last known address is 359 Southwood Drive, Kenmore, New York 14223-1157.

Dr. Rizzo is prohibited from rendering treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rizzo for treatment rendered prior to October 22, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Rizzo regarding treatment he rendered prior to October 22, 2008, should not be denied due to the restriction of his license.
