Please be advised that, by Notice of Revocation, effective December 24, 2008, Dr. Rivero's authorization to render medical treatment and other care was revoked by the Chair pursuant to Workers' Compensation Law section 13-d. Previously, Dr. Rivero's authorization was temporarily suspended, effective March 19, 2007 to March 18, 2008 (see Subject No. 046-181 dated April 6, 2007), which suspension was automatically extended by commencement of the permanent revocation proceeding. Dr. Rivero's current address is 1536 Rockaway Parkway, Brooklyn, New York 11236.

Dr. Rivero is prohibited from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rivero for treatment rendered prior to March 19, 2007, are valid, but are invalid for any treatment on or after that date. Requests to cross-examine Dr. Rivero regarding treatment he rendered prior to March 19, 2007, should not be denied due to the suspension or revocation of his authorization.
