Please be advised that, by Order (Vote and Order No. 23098), effective January 9, 2007, the Board of Regents of the University of the State of New York on behalf of the State Education Department, adopted Dr. Ferguson's application for surrender of his license to practice chiropractic.

Pursuant to Workers' Compensation Law Sections 13–l [12] and 13–d [1], the Chiropractic Practice Committee of the Workers' Compensation Board recommended to the Chair of the New York State Workers' Compensation Board that Dr. Ferguson's authorization to treat injured workers' in the State of New York be revoked, which recommendation was adopted. Effective May 12, 2008, Dr. Ferguson's authorization was formally revoked. Dr. Ferguson's last known address is 285 Gibson Street, Canandaigua, New York 14424.

Dr. Ferguson is prohibited from rendering chiropractic treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Ferguson for treatment rendered prior to January 9, 2007, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Ferguson regarding treatment he rendered prior to January 9, 2007, should not be denied due to the surrender of his license or revocation of his authorization.
