RIMBERTO SANGALANG, M.D. (Authorization No. 148444–3W)

Please be advised that Modification Order (BPMC No. 03–240), effective February 12, 2008, was adopted by the New York State Department of Health, State Board for Professional Medical Conduct ("BPMC"). Therein, Dr. Sangalang agreed to never activate his New York State license registration or reapply for a license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sangalang from the list of authorized physicians, effective April 11, 2008. Dr. Sangalang's last known address is 2227 N. Jerusalem Road, East Meadow, New York 11554.

Dr. Sangalang is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Sangalang for treatment or examinations rendered prior to February 12, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Sangalang regarding treatment or examinations he rendered prior to February 12, 2008, should not be denied due to the inactivation of his license or his removal from the list of authorized physicians.

STEPHEN MICHAEL SHAPIRO, M.D. (Authorization No. 105678–7W)

Please be advised that, by Determination and Order (BPMC No. 08–43), effective April 1, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct ("BPMC") revoked Dr. Shapiro's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Shapiro from the list of authorized physicians, effective April 11, 2008. Dr. Shapiro's last known address is 900 S. Manlius Street, Fayetteville, New York 13066.

Dr. Shapiro is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Shapiro for treatment or examinations rendered prior to April 1, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Shapiro regarding treatment or examinations he rendered prior to April 1, 2008, should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

NICHOLAS F. SAPIENZA, M.D. (Authorization No. 142597–4W)

Please be advised that effective March 27, 2008, the New York State Department of Health, State Board for Professional Medical Conduct ("BPMC") ordered the permanent surrender of Dr. Sapienza's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sapienza from the list of authorized physicians, effective April 11, 2008. Dr. Sapienza's last known address is 31 Fineview Drive, Deerfield, New York 13502.

Dr. Sapienza is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Sapienza for treatment or examinations rendered prior to March 27, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Sapienza regarding treatment or examinations he rendered prior to March 27, 2008, should not be denied due to the surrender of his license or his removal from the list of authorized physicians.

JEFFREY STILLMAN, D.O. (Authorization No. 183994–3B)

Please be advised that, by Consent Order (BPMC No. 08–42) effective March 28, 2008, the New York State Department of Health, State Board for Professional Medical Conduct adopted Dr. Stillman's application for Consent Agreement and Order. Therein, Dr. Stillman did not contest the charges of having been convicted of insurance fraud in violation of the New York State Penal Law Section 176.10.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Stillman from the list of authorized physicians, effective April 11, 2008. Dr. Stillman's last known address is 804 Pratt Drive, Vestal, New York 13850.

Dr. Stillman is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Stillman for treatment or examinations rendered prior to April 11, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Stillman regarding treatment or examinations he rendered prior to April 11, 2008, should not be denied due to his removal from the list of authorized physicians.
