DARRELL W. PONE, M.D. (Authorization Number 157300–5B)
Please be advised that, effective February 20, 2008, Darrell W. Pone, M.D., permanently surrendered to the New York State Department of Health, State Board for Professional Medical Conduct his license to practice medicine in the State of New York pursuant to New York State Public Health Law Section 230.13.
Effective March 10, 2008, Dr. Pone voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 19 Old Westbury Road, Old Westbury, New York 11568–1603.
Dr. Pone is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Pone for treatment or examinations rendered prior to February 20, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Please be advised that, effective January 30, 2008, George Charles Schussler, M.D., permanently surrendered to the New York State Department of Health, State Board for Professional Medical Conduct, his license to practice medicine in the State of New York pursuant to New York State Public Health Law Section 230.13.

By Consent Order (BPMC No. 08-04), effective January 15, 2008, the New York State Department of Health, State Board of Professional Medical Conduct (BPMC) placed Dr. Beckerman on probation for 3 years. Dr. Beckerman did not contest the charge of negligence on more than one occasion.
Effective March 6, 2008, Dr. Beckerman voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 17 Sherwood Drive, Plainview, New York 11803.
Dr. Beckerman is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Beckerman for treatment or examinations rendered prior to March 6, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Beckerman regarding treatment or examinations he rendered prior to March 6, 2008, should not be denied due to this resignation.

By Consent Order (BPMC No. 08-29), effective February 29, 2008, the New York State Department of Health, State Board of Professional Medical Conduct (BPMC) adopted Dr. Arafiles' agreement to never register or reapply for a medical license to practice medicine in the State of New York.
Effective March 7, 2008, Dr. Arafiles voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 706 Champions Row, Victoria, Texas 77904.
Dr. Arafiles is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Arafiles for treatment or examinations rendered prior to February 29, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. By Consent Order (BPMC No. 00-67), effective March 13, 2008, the New York State Department of Health, State Board of Professional Medical Conduct (BPMC) modified the terms of the March 8, 2000, order previously imposed. By modification, Dr. Fogarty has agreed to never activate her registration or reapply for a license to practice medicine in the State of New York.
Effective March 17, 2008, Dr. Fogarty voluntarily resigned her authorization to treat workers' compensation claimants. Her current address is 126 Gilbert Road, New Hartford, New York 13413.
Dr. Fogarty is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Fogarty for treatment or examinations rendered prior to March 13, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Please be advised that, by Surrender Order (BPMC No. 08–33) effective March 7, 2008, the New York State Department of Health, State Board for Professional Medical Conduct ordered the surrender of Dr. Sachs' license to practice medicine in the State of New York.
Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sachs from the list of authorized physicians, effective March 11, 2008. Dr. Sachs last known address is 128 Central Park South, Suite 1D, New York, New York 10019.
Dr. Sachs is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Sachs for treatment or examinations rendered prior to March 7, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Please be advised that, by Surrender Order (BPMC No. 08–21) effective February 14, 2008, the New York State Department of Health, State Board for Professional Medical Conduct ordered the surrender of Dr. Fischberg's license to practice medicine in the State of New York.
Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Fischberg from the list of authorized physicians, effective March 15, 2008.
