Please be advised that, effective February 15, 2008, Hach V. Turyan, M.D., voluntarily resigned his authorization (No. 228887-6W) to treat workers' compensation claimants. His current address is 102–50 62nd Road, Apt. 4N, Forest Hills, New York 11375.

Dr. Turyan is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Turyan for treatment or examinations rendered prior to February 15, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Turyan regarding treatment or examinations he rendered prior to February 15, 2008, should not be denied due to this resignation.
