Subject Numbers Regarding Health Provider Authorizations

RUSSELL PEACOCK, M.D. (Authorization No. 090751-9W)
Please be advised that, by Determination and Order (BPMC No. 07-211) of the New York State Department of Health, State Board for Professional Medical Conduct, effective October 9, 2007, Dr. Russell Peacock's license to practice medicine in the State of New York was limited to preclude all patient contact and any practice of medicine, clinical or otherwise.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Peacock from the list of authorized physicians, effective December 28, 2007. Dr. Peacock's current address is Box 68, South Greenfield Road, Greenfield Center, NY 12833.
Dr. Peacock is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Peacock for treatment or examinations rendered prior to October 9, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Peacock regarding treatment or examinations he rendered prior to October 9, 2007, should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

Please be advised that, by Determination and Order (BPMC No. 07-248) of the New York State Department of Health, State Board for Professional Medical Conduct, effective November 15, 2007, Dr. John Smith's license to practice medicine in the State of New York was suspended until July 1, 2008.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Smith from the list of authorized physicians, effective December 28, 2007. Dr. Smith's current address is 228 W 138th Street, Apt. 2, New York, New York 10030.

Medical reports submitted by Dr. Smith for treatment or examinations rendered prior to November 15, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Smith regarding treatment or examinations he rendered prior to November 15, 2007, should not be denied due to the revocation of his license or his removal from the list of authorized physicians.
