Subject Numbers Regarding Health Provider Authorizations

FRANCISCO JAVIER MONREAL, M.D. (Authorization No. 118576-8B)

Please be advised that, by Determination and Order (BPMC No. 07-141) of the Administrative Review Board of the New York State Department of Health, State Board for Professional Medical Conduct, effective September 11, 2007, Dr. Francisco Javier Monreal's license to practice medicine in the State of New York was revoked.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Monreal from the list of authorized physicians, effective September 11, 2007. Dr. Monreal's current address is 4413 South Salina Street, Syracuse, New York 13205.

Dr. Monreal is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Monreal for treatment or examinations rendered prior to September 11, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Monreal regarding treatment or examinations he rendered prior to September 11, 2007, should not be denied due to the revocation of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 10, 2007, may be provided.

LESLIE DANA WOODCOCK, M.D. (Authorization No. 112637-4W)

Please be advised that, by Consent Order (BPMC No. 07-149) of the New York State Department of Health, State Board for Professional Medical Conduct, effective July 30, 2007, Dr. Leslie Dana Woodcock's license to practice medicine in the State of New York was limited to preclude the practice of medicine, clinical or otherwise.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Woodcock from the list of authorized physicians, effective September 10, 2007. Dr. Woodcock's current address is 77 East Lake Road, Cazenovia, New York 13035.

Dr. Woodcock is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Woodcock for treatment or examinations rendered prior to July 30, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Woodcock regarding treatment or examinations he rendered prior to July 30, 2007, should not be denied due to the limitation of his license or his removal from the list of authorized physicians.

CORDELIA STEVENS, M.D. (Authorization No. 207599-2W)

Please be advised that, by Surrender Order (BPMC No. 07-145) of the New York State Department of Health, State Board for Professional Medical Conduct, effective July 25, 2007, Cordelia Stevens, M.D., surrendered her license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Stevens from the list of authorized physicians, effective September 10, 2007. Dr. Stevens' current address is 735 Kappock Street, Apartment 3D, Bronx, New York 10463.

Dr. Stevens is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Stevens for treatment or examinations rendered prior to July 25, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Stevens regarding treatment or examinations she rendered prior to July 25, 2007, should not be denied due to the surrender of her license or her removal from the list of authorized physicians.
