SAMMER ISLAM FAHMY, M.D. (Authorization No. 206480-6W)

Please be advised that, by Surrender Order (BPMC No. 07-137) of the New York State Department of Health, State Board for Professional Medical Conduct, effective July 9, 2007, Dr. Sammer Fahmy surrendered his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Fahmy from the list of authorized physicians, effective July 10, 2007. Dr. Fahmy's current address is 300 East 40th Street, Suite 18K, New York, New York 10016.

Dr. Fahmy is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Fahmy for treatment or examinations rendered prior to July 9, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Fahmy regarding treatment or examinations he rendered prior to July 9, 2007, should not be denied due to the surrender of his license or his removal from the list of authorized physicians.

STANISLAV KULISHOV, M.D. (Authorization No. 208948-0W)

Please be advised that, by Consent Order (BPMC No. 07-117) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 13, 2007, Dr. Stanislav Kulishov was found to have engaged in professional misconduct and his license to practice medicine was limited to preclude him from treating or billing patients whose services are reimbursed through either No-Fault or Workers' Compensation.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Kulishov from the list of authorized physicians, effective June 21, 2007. Dr. Kulishov's current address is 4100 Ocean Avenue, Brooklyn, New York 11235.

Dr. Kulishov is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kulishov for treatment or examinations rendered prior to June 13, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kulishov regarding treatment or examinations he rendered prior to June 13, 2007, should not be denied due to the limitation of his license or his removal from the list of authorized physicians.

ERNST JEAN PAUL, JR., M.D. (Authorization No. 202775-3W)

Please be advised that, by Consent Order (BPMC No. 07-121) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 15, 2007, Dr. Ernst Jean Paul was found to have engaged in professional or other misconduct.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Paul from the list of authorized physicians, effective July 9, 2007. Dr. Paul's current address is 57 Alta Drive, Mount Vernon, New York 10552.

Dr. Paul is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Paul for treatment or examinations rendered prior to July 9, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Paul regarding treatment or examinations he rendered prior to July 9, 2007, should not be denied due to his removal from the list of authorized physicians.

ANATOLY PISMAN, M.D. (Authorization No. 210766-2B)

Please be advised that, by Consent Order (BPMC No. 07-110) of the New York State Department of Health, State Board for Professional Medical Conduct, effective May 17, 2007, Dr. Anatoly Pisman was found to have engaged in professional misconduct.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Pisman from the list of authorized physicians, effective July 1, 2007. Dr. Pisman's last known address is 1680 Ocean Avenue, #5K, Brooklyn, New York 11230.

Dr. Pisman is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Pisman for treatment or examinations rendered prior to July 1, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Pisman regarding treatment or examinations he rendered prior to July 1, 2007, should not be denied due to his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through June 30, 2007, may be provided.

ALEKSANDER RABUKHIN, M.D. (Authorization No. 207266-8W)

Please be advised that, by Consent Order (BPMC No. 07-114) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 7, 2007, Dr. Aleksander Rabukhin was found to have engaged in professional misconduct and his license to practice medicine was suspended for at least twelve (12) months.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Rabukhin from the list of authorized physicians, effective June 21, 2007. Dr. Rabukhin's current address is 72 Sprucewood Boulevard, Central Islip, New York 11722.

Dr. Rabukhin is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rabukhin for treatment or examinations rendered prior to June 7, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Rabukhin regarding treatment or examinations he rendered prior to June 7, 2007, should not be denied due to his license suspension or his removal from the list of authorized physicians.
