Subject Numbers Regarding Health Provider Authorizations

JOHN E. ZIEGLER, D.C. (Authorization No. C02593-2W)

Please be advised that, effective February 13, 2007, the New York State Education Department, Office of the Professions, granted Dr. John Ziegler's application to surrender his license to practice chiropractic in the State of New York.

Effective June 5, 2007, Dr. Ziegler voluntarily resigned his authorization (Number C02593-2W) to treat workers' compensation claimants. His current address is 301 Saranac Way, Ithaca, New York 14850.

Dr. Ziegler is prohibited from rendering chiropractic treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to February 13, 2007 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Ziegler regarding treatment he rendered prior to February 13, 2007 should not be denied due to the surrender of his license or this resignation.

PHILIP C. DON, M.D. (Authorization No. 172357-6W)

Please be advised that, effective June 10, 2007, Philip C. Don, M.D. voluntarily resigned his authorization (Number 172357-6W) to treat workers' compensation claimants. Dr. Don's office address is 242 Naples Terrace, Bronx, New York 10463.

Dr. Don is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Don for treatment or examinations rendered prior to June 10, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Don regarding treatment or examinations he rendered prior to June 10, 2007 should not be denied due to this resignation.
