FEDERICO C. COLORES, M.D. (Authorization No. 153353-8W)
Please be advised that, by Consent Order (BPMC No. 07-56) of the New York State Department of Health, State Board for Professional Medical Conduct, effective March 20, 2007, Dr. Federico Colores' license to practice medicine in New York State was limited to preclude any patient contact and any practice of medicine, clinical or otherwise.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Colores from the list of authorized physicians, effective April 25, 2007. Dr. Colores current address is 107 Merriam Road, Broadalbin, New York 12025-2217.
Dr. Colores is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Colores for treatment or examinations rendered prior to March 20, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Colores regarding treatment or examinations he rendered prior to March 20, 2007 should not be denied due to the limitation of his license or his removal from the list of authorized physicians.

Please be advised that, by Consent Order (BPMC No. 07-01) of the New York State Department of Health, State Board for Professional Medical Conduct, effective January 10, 2007, Dr. Mark Fleischer surrendered his license to practice medicine in New York State.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Fleischer from the list of authorized physicians, effective April 25, 2007. Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Follette from the list of authorized physicians, effective April 25, 2007. Dr. Follette's current address is 7499 James Street, Fayetteville, New York 13066.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Kark from the list of authorized physicians, effective April 25, 2007. Dr. Kark is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kark for treatment or examinations rendered prior to April 6, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kark regarding treatment or examinations he rendered prior to April 6, 2007 should not be denied due to the limitation of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through April 5, 2007 may be provided.
ABSYLOM K. NYAMEKYE, M.D. (Authorization No. 208330-1W)
Please be advised that, by Commissioner's Order of the New York State Department of Health, State Board for Professional Medical Conduct, effective April 9, 2007, Dr. Absylom Nyamekye has been ordered not to practice medicine in the State of New York or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.
Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Nyamekye from the list of authorized physicians, effective April 25, 2007. Requests to cross-examine Dr. Nyamekye regarding treatment or examinations he rendered prior to April 9, 2007 should not be denied due to the above Order or his removal from the list of authorized physicians.
