PERMANENT REVOCATION — ABRAHAM LOCK, M.D.
On February 13, 2006, Abraham Lock, M.D., surrendered his license to practice medicine to the State Board for Professional Medical Conduct pursuant to a Surrender Order (BPMC No. 06-24). Effective February 15, 2007, Dr. Lock's authorization (Number 166176-8B) to treat workers' compensation claimants and conduct independent medical examinations is permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Lock's last known address is 1801 Avenue J, Brooklyn, New York 11230.
Dr. Lock is permanently prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted for treatment or examinations rendered prior to February 13, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Lock regarding treatment or examinations he rendered prior to February 13, 2006 should not be denied due to the surrender of his license or this revocation. In addition, testimony and/or depositions concerning independent medical examinations conducted through February 12, 2006 may be provided.
VOLUNTARY RESIGNATION — ANDREW L. ORLANDER, D.C.
Please be advised that, effective October 24, 2006, Andrew L. Orlander, D.C. surrendered his license to practice chiropractic medicine in New York State to the State Board of Regents (License No. 003499).
Effective February 22, 2007, Dr. Orlander voluntarily resigned his authorization (Number C03499-3W) to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 11 Tall Pines Drive, Bedford Corners, New York 10549.
Dr. Orlander is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Orlander for treatment or examinations rendered prior to October 24, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Orlander regarding treatment or examinations he rendered prior to October 24, 2006 should not be denied due to the surrender of his license or this resignation.
VOLUNTARY RESIGNATION (IME's ONLY) — YUSUF M. DINCER, M.D.
Please be advised that, effective January 31, 2007, the State Board for Professional Medical Conduct issued a Consent Order (BPMC No. 07-13) in regard to Yusuf M. Dincer, M.D.'s license to practice medicine in New York State, which resulted in a 3 year probationary term.
Effective February 21, 2007, Dr. Dincer voluntarily resigned his authorization (Number 200366-3B) to conduct independent medical examinations of workers' compensation claimants. His current business address is 319 South Manning Boulevard, Suite 202, Albany, New York 12208.
Dr. Dincer is prohibited from conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Dincer for independent medical examinations conducted prior to February 21, 2007 are valid, but are invalid for any independent medical examinations conducted on or after that date. Testimony and/or depositions concerning independent medical examinations conducted through February 20, 2007 should not be denied due to the Consent Order or this resignation.
Dr. Dincer maintains his authorization to render medical or other treatment and care to individuals who have suffered work-related injuries or illnesses under authorization number 200366-3W.
