VOLUNTARY RESIGNATION – STUART PRESS, M.D.
Please be advised that, effective December 26, 2006, Stuart Press, M.D. surrendered his license to practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license to the State Board for Professional Medical Conduct pursuant to a Surrender Order (BPMC No. 06-296).
Effective January 24, 2007, Dr. Press voluntarily resigned his authorization (Number 144098-1W) to treat workers' compensation claimants. Dr. Press's current address is: 1525 East Putnam Avenue, Old Greenwich, Connecticut 06870.
Dr. Press is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Press for treatment or examinations rendered prior to December 26, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Press regarding treatment or examinations he rendered prior to December 26, 2006 should not be denied due to the surrender of his license or this resignation.

Please be advised that, effective December 28, 2006, Alberto Beltrano, M.D. surrendered his license to practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license to the State Board for Professional Medical Conduct pursuant to a Surrender Order (BPMC No. 06-301).
Effective January 23, 2007, Dr. Beltrano voluntarily resigned his authorization (Number 153336-3W) to treat workers' compensation claimants. Dr. Beltrano's current address is: 41 Cherry Street, Lyons, New York 14489.
Dr. Beltrano is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Beltrano for treatment or examinations rendered prior to December 28, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Beltrano regarding treatment or examinations he rendered prior to December 28, 2006 should not be denied due to the surrender of his license or this resignation.
TEMPORARY SUSPENSION – GEORGE KOTLEWSKI, M.D.
Please be advised that, effective January 12, 2007, George Kotlewski, M.D. consented to a temporary suspension of his authorization (Number 118551-1B) to treat and conduct independent medical examinations of workers' compensation claimants pursuant to Workers' Compensation Law §13-d(4) by executing a Consent Agreement to that effect with the Workers' Compensation Board. Dr. Kotlewski's address is 8349 Black Walnut, East Amherst, New York 14051. The suspension remains in effect until the 3 year period of probation pursuant to Consent Order (BPMC #06-101), which became effective May 10, 2006, has been successfully completed and satisfactory proof of completion has been provided to the Board.
While the suspension is in effect, Dr. Kotlewski is prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kotlewski for treatment or examinations rendered prior to January 12, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kotlewski regarding treatment or examinations he rendered prior to January 12, 2007 should not be denied due to this suspension. In addition, testimony and/or depositions concerning independent medical examinations conducted through January 11, 2007 may be provided.
