Subject Numbers Regarding Health Provider Authorizations

VOLUNTARY RESIGNATION – DR. VLAD A. MEISHER, M.D.

Please be advised that effective August 22, 2006, Vlad A. Meisher, M.D., voluntarily resigned his authorization (Number 186086-5W) to treat workers' compensation claimants. Dr. Meisher's last known office was located at 36-19 Hillside Terrace, Fair Lawn, New Jersey 07410. His license to practice medicine in the State of New York was surrendered, by agreement, to the Department of Health, Board for Professional Medical Conduct, effective July 21, 2006.

Dr. Meisher is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Meisher for treatment rendered prior to July 21, 2006, are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Meisher regarding treatment he rendered prior to July 21, 2006 should not be denied due to this resignation.

VOLUNTARY RESIGNATION – DR. LOUIS ANTHONY SIDOTI, M.D.

Please be advised that effective August 23, 2006, Louis Anthony Sidoti, M.D., voluntarily resigned his authorization (Number 186890-0W) to treat workers' compensation claimants. Dr. Sidoti's last known office address is One Sunrise Terrace, Wynantskill, New York 12198. His license to practice medicine in the State of New York was revoked by the Department of Health, Board for Professional Medical Conduct, effective August 11, 2006.

Dr. Sidoti is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Sidoti for treatment rendered prior to August 11, 2006, are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Sidoti regarding treatment he rendered prior to August 11, 2006 should not be denied due to this resignation.
