Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective August 8, 2006, Herbert Louis Gould, M.D., voluntarily resigned his authorization (number 083214-7B) to treat and to conduct independent medical examinations of workers' compensation claimants. Dr. Gould's last known office was located at 90 Greenridge Avenue, White Plains, New York 10605-2418.

Dr. Gould is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gould for treatment or examinations rendered prior to August 8, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Gould regarding treatment or examinations he rendered prior to the resignation's effective date should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through August 7, 2006, may be provided.
