Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective April 28, 2006, Benjamin E. Zola, M.D., voluntarily resigned his authorization (number 148544-0W) to treat workers' compensation claimants. Dr. Zola's last known office address is 210 East 86th Street, Room 502, New York, New York 10028-3003.

Dr. Zola is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Zola for treatment rendered prior to April 28, 2006, are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Zola regarding treatment he rendered prior to the resignation's effective date should not be denied due to this resignation.
