Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective December 27, 2005, Dr. Arnaldo Francesco Trabucco's authorization (number 166371-5) to treat workers' compensation claimants has been temporarily suspended pursuant to Workers' Compensation Law §§ 13-b and 13-d(4). Dr. Trabucco's last known address is 1549 North Meadows Court, Columbus, Indiana. The suspension is effective until December 26, 2006.

Dr. Trabucco is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses while this suspension is in effect. Medical reports submitted for treatment and care rendered prior to December 27, 2005 are valid, but are invalid for any treatment or care rendered on or after that date. Requests to cross-examine Dr. Trabucco regarding treatment and care he rendered prior to the suspension's effective date should not be denied due to this suspension, and the physician is entitled to appropriate payment.
