Subject Number 046-1318 Removal of Dr. Robert Edmund White

Subject Numbers Regarding Health Provider Authorizations

Authorization No. 163661-2B

August 5, 2020

Pursuant to Workers' Compensation Law Section 13-d(2)(a), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Robert Edmund White’s authorizations to treat injured workers and to perform independent medical examinations in the New York State workers’ compensation system effective July 29, 2020. Dr. White's last known business addresses are:

124-16 84th Road, Apt. 2L
Kew Gardens, New York 11415

and

11905 80th Road
Kew Gardens, New York 11415

The Chair’s designee’s investigation has revealed that by Consent Order BPMC 20-182, effective July 29, 2020, in full satisfaction of the charges against him, Dr. White agreed to an order precluding him from the practice of medicine in New York State, based on allegations that during the periods from in or about 2012 through in or about 2017, he failed to render and/or note appropriate care and treatment to multiple patients, and prescribed controlled substances in an inappropriate manner to these patients.

Therefore, the Chair believes that Dr. White may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.

Dr. White is prohibited from rendering treatment and care to injured workers, from performing independent medical examinations, and from conducting reviews of records regarding variance requests pursuant to 12 NYCRR § 324.3(b)(2)(ii)(a). Reports submitted by Dr. White for services rendered prior to July 29, 2020 are valid but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. White for services rendered prior to July 29, 2020, should not be denied due to the removal of his authorizations.

Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
