Pursuant to Workers' Compensation Law Sections 13-b and 13-d(2), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Helena Kukla’s authorization to treat injured workers in the New York State workers’ compensation system effective November 18, 2019. Dr. Kukla's last known business address is:

The removal is based upon Order of Conditions (BPMC No. 19-277), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective November 21, 2019, wherein Dr. Kukla has been precluded from practicing medicine in New York State, from practicing in any setting where her practice is based solely on her New York license, and from further reliance on her New York license to practice medicine.

Dr. Kukla is prohibited from rendering treatment and care to injured workers, and from conducting reviews of records regarding variance requests pursuant to 12 NYCRR § 324.3(b)(2)(ii)(a). Reports submitted by Dr. Kukla for services rendered prior to November 18, 2019, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Kukla for services rendered prior to November 18, 2019, should not be denied due to the removal of her authorization.
