Subject Numbers Regarding Health Provider Authorizations

Please be advised that Dr. Jeffrey A. Spina has resigned his authorizations to treat injured workers and to perform independent medical examinations in the New York State workers’ compensation system effective September 21, 2018. Dr. Spina’s last known practice addresses are:

and

201 Dolson Avenue, Suite G
Middletown, New York 10940

Dr. Spina was temporarily suspended from the New York State workers’ compensation system, effective September 17, 2018 (see Subject Number 046-1102).

Dr. Spina is prohibited from rendering treatment and care to injured workers and from performing independent medical examinations. Reports submitted by Dr. Spina for services rendered prior to September 17, 2018, (the date of Dr. Spina’s temporary suspension) are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Spina for services rendered prior to September 17, 2018, should not be denied due to the resignation of his authorizations.
