Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. Nasim Haider’s authorization to treat injured workers within the New York State workers’ compensation system effective February 16, 2018. Dr. Haider's last known business address is:

Dr. Haider’s authorization to treat injured workers was temporarily suspended by the Board effective February 13, 2017.
The revocation is based upon Administrative Review Board for Professional Medical Conduct (ARB) Determination and Order No. 18-029, issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective February 14, 2018, wherein Dr. Haider’s license to practice medicine in New York State was revoked for practicing medicine fraudulently, practicing medicine with negligence on more than one occasion, engaging in conduct in the practice of medicine that evidences moral unfitness, willfully making or filing a false report, or failing to file a report required by law or by the Department of Health or the Education Department, violating a term of probation, condition or limitation imposed on a license pursuant to PHL §230, willfully harassing or abusing a patient either physically or verbally, failing to maintain adequate medical records, and failing to use accepted infection control practices pursuant to PHL §230(a).
Dr. Haider is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Haider for services rendered prior to February 13, 2017 are valid but are invalid for any services rendered on or after that date.
