Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. Mohamed Abdel Hakeem Khalaf’s authorization to treat injured workers within the New York State workers’ compensation system effective February 1, 2018. Dr. Khalaf's last known business addresses are:

and

Medina, New York 14103

The revocation is based upon Determination and Order (BPMC No. 18-006), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective January 15, 2018, wherein the charges against Dr. Khalaf of gross negligence, gross incompetence, negligence on more than one occasion, incompetence on more than one occasion, failure to maintain records, improper delegation, and fraudulent practice were sustained, based on multiple patients for whom he provided gynecological, prenatal, and obstetrical care. Pursuant to said order, Dr. Khalaf’s license has been revoked.

Dr. Khalaf is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Khalaf for services rendered prior to February 1, 2018 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Khalaf for services rendered prior to February 1, 2018 should not be denied due to the revocation of his authorization.
