Please be advised that, effective August 9, 2010, Charles Anderson, M.D., voluntarily resigned his authorization to provide treatment and care to individuals who have suffered work-related injuries and illnesses. Dr. Anderson's business address is 191 North Street, Buffalo, New York 14201.

This action supersedes Subject Number 046-439 in which it was noted that Dr. Anderson's authorization to treat was suspended from July 26, 2010, through July 25, 2011. The suspension is therefore amended to reflect the voluntary resignation. The suspension period is July 26, 2010, through August 8, 2010, and effective August 9, 2010, Dr. Anderson voluntarily resigned his authorization.

Dr. Anderson is prohibited from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Anderson for treatment rendered prior to July 26, 2010 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Anderson regarding treatment he rendered prior to July 26, 2010, should not be denied due his temporary suspension or his voluntary resignation from the list of authorized physicians.
