Please be advised that, effective September 24, 2009, Bashir A. Zikria, M.D., voluntarily resigned his authorization to conduct independent medical examinations of individuals who have suffered work-related injuries or illnesses. No business address is available for Dr. Zikria.

Dr. Zikria is prohibited from conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Zikria for examinations conducted prior to September 24, 2009, are valid, but are invalid for any examinations conducted on or after that date. Requests to cross-examine Dr. Zikria regarding examinations he conducted prior to September 24, 2009, should not be denied due to the resignation of his authorization.
